Don't get the machining option from ESR if you get this carb. The stock thumb throttle cable will mate with the 38mm. If you have a heat gun, warm up the intake boot to fit it over the carb bell (it also doesn't hurt to lube the bell housing with some 3 in 1 oil or wd-40. You will also need to heat up/lube the rear bell to get the airbox boot over the carb. Get some extra pilot jets 42-52 and get some extra main jets 160-172.

Stick with a Keihin PJ carb. 1986 stock 34mm
Or a Keihin PJ carb 36mm or 38mm
A Keihin PJ carb off a 1986 to 1998 Honda CR 250 38mm will have the closet jetting. (There are many bikes that came with those carbs, the CR250 has the closest jetting stock)
There are also many newer Keihin carbs and other brands of carbs that work great, but if you are having trouble with the stock carb...keep it simple.

From what I have seen and talking to Mr. James on the phone, he seems to be more of a restorer (And a excellent one at that) then a hot rodder.
I am surprised it doesn't have a stock pipe & silencer.

Maybe with your pipe, silencer, and other mods you need to change your jetting to accommodate more airflow. The air/fuel ratio probably isn't quite right. Rebuilding your carb or swapping out jets is way cheaper than getting a new carb. But if you do decide to just get a new carb for it and not have to worry about trouble shooting your stock carb. I would recommend the keihin pwk 38mm.

I was in the same boat as you with my 85R and followed yaegerb'is advice. I got the Airstriker 38mm and stretched my boots to fit. That part wasn't necessarily easy, but it was a breeze to jet and tune.
It made like a LOT easier and am very happy with this upgrade.
