What kind of bike? A little more infor would be great. Could be bad bearings in the rearor a loose axel . Swing arm looseor bad bearings. If it it just cracked and not split you can get it welded or jb weld could work if it is just a leak.

    We want pics of both.....

    To answer your questions:

Chains fall off when sprockets get severely worn, chain rollers and sliders deteriorate and when axle or pivot bearings get worn.

If you use a dremel, some acetone and sandpaper; You can rough up most cracks etc and try some epoxy.

Obviously it's better to figure out whats making the chain launch and then replace the case. But, thats the most expensive part and everyone's broke so have at the epoxy. I have a motor I'm fixing with epoxy right now......

jb weld will work, the surface must be oil free and it must be mixed evenly and allowed to cure. my 200s had a hole in the crank case from the chain and it held up the entre time i owned it wich was 8 months. it looked like it seeped a bit of oil but just tiny amounts nothing to get worried about

also make sure you have the chain tensioners in place and make sure you have the right bolts on the carrier, or else its a lost cause

You could try JB Weld or there's a new product out that is as good, or better: http://www.rapid-fix-glue.com/ A friend of mine got it in at his auto parts store a couple months back. It's a bit pricey ($28 for both bottles) but it's still cheaper than a new case. Check the video on that link. It's pretty cool stuff.

loose chains are the cause of this,if your chain comes off once get it home and fix it properly.change sprockets and chain together when needed.cases get expensive.
