I added a grease zerk to my carrier (first generation atc200x) I plan on removing the inside grease/dust cover from the inside of the new bearings I just ordered so I can pump in some waterproof grease when installed, should I also drill some holes into the metal sleeve that surrounds the axle? Am I over thinking this?

If you installed a zerk fitting in your swingarm, you will need to install a few large hole in the bearing/axle holder to get grease to them because the cavity between the bearing/axle holder and the swingarm housing is also going to fill with grease and come out the slots where the bolts to tighten it on the bearing/axle carrier are . . The large holes in the bearing/axle carrier will allow more grease to go in their than on the ground thru the slots, lol.

On the first generation there is no gap to tighten a carrier in the swingarm. The bearings go into the swingarm directly so the grease will be trapped in by the outer bearing sells and dust seals. No way for it to get out.

Ok, got ya, sorry, I had the wrong bike in my head . . Yes your plan will work but I would be a little concerned about some grease getting past the seals or something on the brake drum side then getting into the brakes, so if you do this, I would consider checking that area after you fill it up and then maybe again after you ride it for a while..

Runmedown, I have done the same thing you are doing to darn near all the trikes I have owned. I just got doing the same to my 300EX trike conversion since I had to put new carrier/bearings in it.
I have always left the outside seals in place,,, pretty much inline with barnetts thought process. Any excess grease will find a way out, ya going to hafta trust me on that one,,, it will.
And yes, drill holes in the bearing spacer (some take a cobolt drill to get hole in ~ very hard metal. Can't remember how my air cooled R was or any of the 200x's I used to own was,,, hard or just regular steel)

I have done the same thing you are describing to all of my first gens with no problems....I also use the same method on the swing arm bearing/pivot bolt area for easy bearing maintenance.
Don't worry...just run watcha brung...

I drilled out through bearing spacer, it ready to go. Would anyone advise doing the same on the swingarm bolt side?

I don't understand what you are saying, but if the bearing spacer butts up against the inner bearing races, there is no point in drilling it out because that is not where the bearings are.

Yes, do the swingarm pivot bolt too.
The 350x I added zirks too was easy in regards to swing arm pivot bolt. It was already hollow. Bored some holes in it and drilled tapped threads in ends to add zirks.

A little grease between the axle and spacer never hurts either. I just repaired a front axle on an 85 250ES that the spacer was rusted to the axle. :O I had to cut the spacer off with a 4in grinder and make a new one. I always grease up the shaft before I stick it in.
