Honistly.. bad luck. Sometimes things happen that have no real explanation. Old parts that sometimes take years to show a flaw thats been there all along. As long as a gear was never siezed on that shaft, and both shaft bearings had little or no play. Nothing could have been done short of magnufluxing shaft to reveal a potential failure.
